At a senior care center in Shanghai’s Xuhui District, a nurse fastens a slim white sensor around the fragile wrist of an 82-year-old woman. The device tracks her heartbeat, records her movements, and sends real-time alerts to staff in case she falls. Down the hallway, a wall of monitors blink with data: oxygen levels, motion graphs, and digital heartbeats pulsing red.
No family members visit. Only machines keep watch.
All across China, scenes like this have become common as the country turns to automation and artificial intelligence to fulfill the role once occupied by family caregivers. From wearable sensors to delivery robots, “smart eldercare” facilities now anchor the nation’s aging strategy. It’s a transformation driven less by choice than by demographic statistics that have made traditional caregiving impossible.
“China is the epicenter of aging,” explains Pinchas Cohen, dean of the USC Leonard Davis School of Gerontology. Additionally, he remarks that there are more seniors in China than there are people in the entire United States, with a ratio of elderly to working-age adults higher than anywhere else in the world.
The country operates 1.2 million beds in senior living facilities compared to India’s 50,000, despite having a similar population size. Yet even this massive infrastructure can’t keep pace. “There are just not enough people to take care of older folks, even if there’s housing for them,” Cohen says.
This shortage stems from decades of demographic engineering through the one-child policy. The one-child policy, enforced from 1980 to 2015, created a generation of only children now responsible for caring for two parents and often four grandparents. Demographers call it the
4-2-1 problem. China’s population shrank for the third consecutive year in 2024, a trend that will only accelerate. The mathematics here have made traditional filial piety impossible.
Cohen traced the cultural disconnect back to the economic forces at play. Chinese society as a whole “used to rely on intergenerational aging place models, which dramatically changed as industrialization accelerated over the last three decades.” Younger Chinese people migrated from rural areas to big cities for work, often leaving their aging parents in rural villages without a set of support systems. The Confucian ideal of children caring for their parents at home collapsed under the weight of factory schedules and urban apartment living.
Into this gap, technology arrived with the promise of scalability. In late 2024, the Ministry of Industry and Information Technology, in collaboration with the Ministry of Civil Affairs, launched a national robot pilot program for elderly care. The initiative directs billions of yuan toward AI-powered monitoring systems, companion robots, and rehabilitation devices. The program focuses on three areas: home care, community support, and institutional settings.
Applications range from assisting people with disabilities to providing emotional support and enabling smart homes. “They’re now leading in terms of robotics and other approaches to caregiving,” Cohen says.
The government’s approach borrows from Japan, where robots have been integrated into elder care for years. Devices like Robear, a bear-shaped machine that helps seniors sit and stand, and Paro, a furry robotic seal that provides companionship, have become familiar sights in Japanese nursing homes. China hopes to replicate this success at a far larger scale, with more than 100 Chinese robotics startups now specializing in rehabilitation, companionship, and nursing robots.
The results, so far, are mixed. In Shenyang, capital of northeast China’s Liaoning Province, a 70-year-old stroke survivor named Zhang regained his ability to walk using an exoskeleton rehabilitation device developed by SIASUN Robot and Automation Co. “At first, I could barely lift my leg,” he says. “But now, I can climb stairs again.” The technology gave him back his mobility when human therapists were in short supply, a success story the government has been eager to promote.
At the First Social Welfare Home in Chongqing, an 86-year-old woman named Wang has grown fond of Peipei, a humanoid robot programmed for emotional companionship. Wang chats with Peipei, plays games, and even asks the robot to photograph her and smooth out the wrinkles digitally. “If you have any questions, just ask ‘Peipei’. She can answer anything,” Wang says. The robot notices when Wang hasn’t slept well or seems upset. These are gestures that feel, to Wang, like care.
At Shenzhen Nursing Home, robots have become part of daily life, playing chess with residents and offering AI-assisted therapy. One elderly resident, watching a humanoid robot demonstrate its capabilities, remarked hopefully: “I’m so happy to see robots becoming part of our lives. I hope they bring us more convenience.” The statement captures both the promise and the uncertainty of this technological shift.
But the industry faces significant obstacles to scaling up. Robot prices remain prohibitively high for many facilities. Chinese nursing homes often struggle to turn a profit, with most needing occupancy rates above 90 percent to break even. Few have cash available to invest in expensive technology. The co-founder says he wants to install smart mattresses and bathing robots to reduce pressure on his overworked staff, but the center isn’t yet profitable. With only 30 residents in a facility designed for 100, he can’t justify raising monthly fees from 2,000 to 3,000 yuan to cover the costs.
Cohen acknowledged that there is tension behind the progress. China’s efficiency in identifying problems and deploying solutions at scale raises uncomfortable questions about what gets lost in translation. Can technological care ever replace a sense of organic human connection? A 2023 study of 207 Chinese older adults found that acceptance of socially assistive robots stood at just 51 percent, with many elderly people expressing discomfort with the technology’s inability to understand local dialects or provide genuinely emotional responses.
“One thing about China is that they care a lot about longevity. They value their elders and make sure, one way or another, that their older parents and grandparents are being taken care of. That’s a value I respect.”
This transformation seems to be changing the means of care. It is not as much a moral failure as it is an economic consequence of modernization. Cohen added that aging is a global issue and that the USC Leonard Davis School trains more Chinese students in gerontology than any other academic program in the world. “They come here to learn how to design systems that balance dignity and efficiency,” he said. “Aging is a concern everywhere, but a really big issue in China.”
However, even advocates acknowledge the limitations of technology. Chen Zhengxiang, an AI model trainer who has conducted research in nursing homes across the country, points out that current robots struggle to recognize commands spoken in local dialects. Their emotional interactions remain at what he calls “a superficial, programmed response level.” He envisions a future where
robots can genuinely perceive and respond to human emotions, providing what he calls “an ‘AI child’ by your side.” But that future, he admits, has not yet arrived.
In China, the challenge is both demographic and philosophical: how does one care for millions without sacrificing the human touch that elderly care demands? Machines can monitor breathing and movement. But they cannot hold a hand with human compassion.
